El aparato tiene que amplificar los suficiente como para lograr que la intensidad sonora que proporciona esté por encima del umbral de audición del paciente, pero sin llegar a un volumen capaz de desencadenar el tan desagradable reclutamiento. Si este se produce el paciente tolera mal el audífono, se queja de que no puede soportarlo cuando le hablan fuerte o cuando no existe un ambiente con alto nivel sonoro, dice que le retumba, que le hace ruidos, y de manera progresiva o inmediata, va prescindiendo de su utilización.

El aparato debe comprimir su volumen dentro de la banda útil de audición de su propietario. Labanda útil es el campo sonoro existente entre el umbral de audición y el límite sonoro en que aparece el reclutamiento.

En cualquier amplificador se denomina ganancia a la diferencia entre la intensidad de la señal de entrada y la intensidad de la señal de salida (diferencia entre input y output). La señal de salida nunca deberá llegar al límite sonoro en que comienza el reclutamiento, para ello se debe realizar una limitación de la salida, preparando el circuito de manera en que la unidad de compresión se active a una intensidad de sonido determinada, con lo que se pone en marcha en milisegundos un control de volumen.

El nivel de inteligibilidad y la regresión fonémica son factores muy a tener en cuenta en el momento de la indicación y la adaptación de un audífono. Es frecuente en distintas hipoacusias neurosensoriales y típico de la presbiacusia el hecho de que coexista un umbral muy aceptable para la audición de los tonos puros en la audiometría con una notable dificultad para percibiry comprender el lenguaje hablado en la vida normal. Estos paciente nos dicen que “oyen pero no entienden”. En estas personas es inútil amplificar linealmente los sonidos externos mediante una prótesis, lo único que lograremos con ello es proporcionarles molestias con sensaciones sonoras desagradables. Es un problema complicado que se logra mediantemicrófonos direccionales que recogen preferentemente los sonidos producidos delante del usuario y por el filtrado de la energía sonora de baja frecuencia, que es el componente fundamental del ruido ambiente y participa poco en la comunicación oral.

 

La utilización de dos audífonos es lo ideal en el mayor número de pacientes. En la hipoacusia infantil es imprescindible la adaptación en ambos oídos.

 

En la indicación, adaptación y aceptación de un audífono entra en juego muy variadas 
connotaciones personales, sociales, psicológicas, etc.

 

Pacientes que teóricamente debían obtener un beneficio muy gratificante con el uso de esta ayuda protésica, muestran sin embargo escepticismo e incluso clara resistencia para su empleo.
